Appointments are subject to availability. Clients are responsible for providing accurate information during scheduling and arriving on time with any required identification or documentation.
Payment is due at the time of service unless other arrangements have been made. Fees are subject to change without prior notice.
Missed appointments or cancellations may be subject to fees. Services that have already been performed, including specimen collection, laboratory submissions, or background screening requests, are generally non-refundable.
