Square One Lab Testing Solutions LLC is committed to providing accurate, confidential, and professional specimen collection and occupational health services. Because many of our services involve time-sensitive specimen collection, laboratory processing, chain-of-custody procedures, and third-party laboratory fees, the following policies apply to all clients.
All sales are considered final once any of the following occurred:
A specimen has been collected.
A laboratory test has been initiated.
A DNA collection kit or laboratory kit has been shipped.
Fingerprinting services have been completed.
A background screening request has been submitted.
Any third-party laboratory or service provider has begun processing your order.

DOT & Non-DOT Drug Testing
Hair Follicle Drug Testing
Oral Fluid (Saliva) Drug Testing
Breath Alcohol Testing
Fingerprinting Services
Background Check Facilitation
Mobile Phlebotomy Services
Occupational Health Collection Services
If you need to cancel or reschedule an appointment, please notify us at least 24 hours prior to your scheduled appointment.
Appointments cancelled with at least 24 hours’ notice and before any specimen has been collected, laboratory services initiated, or testing kits shipped may qualify for a partial refund, less a $25 administrative processing fee.
Clients who fail to appear for a scheduled appointment without prior notice (“No-Show”) will forfeit any payments made and will be required to pay the full-service fee to schedule another appointment.
Once a home DNA collection kit, laboratory collection kit, or other testing materials have been shipped, the order is considered final.
Shipping charges are non-refundable under all circumstances, regardless of whether the kit is opened, used, or returned.
To maintain the privacy, safety, security, and efficiency of our collection site, only individuals scheduled for testing are permitted inside the testing area.
Visitors, children, family members, or additional guests are not permitted unless:
They are the parent or legal guardian of a minor being tested.
They are required by law or court order.
They have received prior approval from Square One Lab Testing Solutions LLC due to medical accommodation or disability.
For employer-requested testing, all results are released directly to the employer’s Designated Employer Representative (DER), Human Resources department, Medical Review Officer (MRO), or other authorized company representatives.
Square One Lab Testing Solutions LLC does not release employer-requested drug or alcohol testing results directly to employees or donors.
When requested, DOT and non-DOT testing information may also be provided to authorized Substance Abuse Professionals (SAPs), Third-Party Administrators (TPAs), Medical Review Officers (MROs), or other compliance partners involved in the testing process.
Employees with questions regarding workplace testing should contact their employer.
Legal DNA test results are released only to the authorized paying party of record or other individuals legally entitled to receive the results.
Results are delivered securely according to the laboratory’s approved reporting procedures.
All participants must present valid government-issued photo identification before specimen collection.
If services are provided under an approved payment arrangement, all outstanding balances must be paid in full before test results are released.
Square One Lab Testing Solutions LLC will not release DNA results to grandparents, spouses, partners, relatives, attorneys, or any other third party without appropriate legal authorization, court order, or written consent from the individual legally authorized to receive the results.
Mileage and travel fees for mobile appointments are due prior to the scheduled appointment unless alternative billing arrangements have been established.
Additional travel fees may apply for extended service areas, after-hours appointments, weekends, holidays, or emergency service requests.
An inconclusive laboratory result means that the laboratory was unable to issue a definitive report due to circumstances such as:
Insufficient specimen quantity
Specimen integrity concerns
Laboratory processing limitations
Collection issues beyond the laboratory’s control
Other technical factors preventing analysis
When an inconclusive result occurs, the employer, Designated Employer Representative (DER), Medical Review Officer (MRO), Human Resources representative, or other authorized contact will be notified as appropriate.
Square One Lab Testing Solutions LLC does not release employer testing information directly to employees or donors.
When necessary, a recollection may be required.
Additional fees for recollection, laboratory processing, travel, or mobile services may apply unless otherwise specified in a written service agreement.
Square One Lab Testing Solutions LLC is not responsible for delays resulting from laboratory review, regulatory requirements, shipping carriers, or circumstances beyond our reasonable control.
If a payment dispute or chargeback is initiated without first contacting Square One Lab Testing Solutions LLC, we reserve the right to:
Suspend current or future services.
Assess reasonable administrative costs associated with the dispute.
Refer unpaid balances to a collection agency.
Pursue legal remedies where permitted by law.
We encourage clients to contact us directly regarding any billing concerns so we may work toward a timely resolution.
